Job Summary:
Provide outpatient physical therapy services to patients, focusing on recovery, mobility, and functional independence. Assess patient needs, develop treatment plans, and document progress in accordance with regulatory standards.

Responsibilities:

• Evaluate patients’ physical conditions and establish personalized treatment goals.
• Develop and implement targeted therapy plans to improve mobility and strength.
• Monitor patient progress and adjust treatment strategies as needed.
• Educate patients and their families on exercises and safety precautions for ongoing recovery.
• Maintain accurate, timely documentation of all therapy sessions and patient outcomes.
• Collaborate with medical teams and report on patient status during care plan reviews.

Qualifications:

• Current state licensed Physical Therapist credential.
• Minimum of 1 year of recent outpatient or home health physical therapy experience.
• Proven ability to evaluate and treat a diverse patient population.
• Strong documentation skills and familiarity with EMR systems.
• Excellent communication and patient education skills.
• Ability to work independently and manage a flexible schedule.
