Job Summary:

Seeking a dedicated Physical Therapist to provide home health services within Prince George's County and Charles County. The ideal candidate will have home health experience and be committed to delivering quality patient care in a community setting.

Responsibilities:

• Assess and develop individualized treatment plans for adult patients in a home health environment.
• Administer physical therapy interventions to improve or restore patient mobility and function.
• Document patient progress and update care plans accordingly.
• Collaborate with healthcare teams to coordinate patient care and ensure compliance with all regulations.
• Adhere to visit schedules, typically completing six visits per day.
• Maintain accurate records using HCHB and other relevant documentation systems.

Qualifications:

• Hold an active MD PT State license at the time of submission.
• Possess at least one year of experience in a home health setting.
• Be certified in BLS with valid certification verification.
• Exhibit strong communication and documentation skills.
• Be flexible to work Monday through Friday, 8:00 AM to 5:00 PM.
• Demonstrate ability to work independently and within compliance protocols.
