Job Summary:
Provide experienced occupational therapy services to patients in skilled nursing facilities, focusing on improving their daily functioning and independence. Collaborate with healthcare teams to develop and implement individualized treatment plans.

Responsibilities:

• Assess patients' physical, emotional, and cognitive abilities to determine therapy needs.
• Design and execute tailored treatment programs to promote recovery and independence.
• Document patient progress and update treatment plans regularly.
• Educate patients and family members on therapeutic exercises and strategies.
• Maintain compliance with all licensing, safety, and regulatory standards.
• Use NetHealth (Rehab Optima) system to record and track patient data accurately.

Qualifications:

• Possess at least one year of experience working in a Skilled Nursing Facility (SNF).
• Hold an active California occupational therapist license.
• Demonstrate proficiency in using NetHealth (Rehab Optima) software.
• Strong knowledge of occupational therapy practices and interventions.
• Effective communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
