Job Summary:

Provide specialized physical therapy services to patients in a skilled nursing facility setting. Promote rehabilitation and functional recovery through evidence-based therapeutic interventions.

Responsibilities:

• Assess patients' physical conditions and develop tailored treatment plans.
• Implement therapeutic exercises and modalities to improve mobility and strength.
• Monitor patient progress and adjust treatments accordingly.
• Document clinical assessments, interventions, and outcomes accurately.
• Collaborate with interdisciplinary team members to coordinate patient care.
• Educate patients and families on therapeutic exercises and injury prevention strategies.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• BLS certification required.
• Demonstrate clinical proficiency in adult rehabilitation and acute care settings.
• Exhibit excellent communication and interpersonal skills.
• Possess active clinical license and credentialing documentation.
• Prior experience in SNF or long-term care preferred.
