Job Summary:
Provides specialized occupational therapy services to patients in a home health setting, focusing on enhancing independence and functional ability. Collaborates with healthcare teams to develop and implement treatment plans tailored to individual patient needs.

Responsibilities:

• Assess patients' condition and determine appropriate occupational therapy interventions.
• Develop personalized treatment plans to improve patient functionality and independence.
• Deliver direct therapy services, including therapeutic activities and exercises.
• Monitor and document patient progress accurately and timely.
• Educate patients and caregivers on therapeutic strategies and home exercises.
• Maintain compliance with clinical protocols and regulatory standards.

Qualifications:

• Hold a valid Occupational Therapist license in the state of practice.
• Possess at least 1 year of occupational therapy experience, preferably in home health.
• Demonstrate proficiency in Oasis documentation and therapy software.
• Be BLS certified through AHA.
• Have an active driver’s license and personal transportation.
• Exhibit strong communication and documentation skills.
