Job Summary:

Provide comprehensive occupational therapy services to patients, promoting independence and functional ability through assessment and tailored treatment plans. Collaborate with healthcare teams to deliver patient-centered care in a home health setting.

Responsibilities:

• Evaluate patients' functional abilities and develop individualized treatment plans.
• Administer therapeutic interventions to improve patients’ daily living skills.
• Document patient progress and maintain accurate records in compliance with regulatory standards.
• Communicate effectively with patients, families, and healthcare professionals regarding care plans and progress.
• Ensure adherence to all safety and compliance guidelines during therapy sessions.
• Participate in on-call rotation and coordinate visits efficiently based on patient needs.

Qualifications:

• Hold an active CA State Occupational Therapist license or pending application.
• Possess at least 1 year of relevant home health experience, with Oasis experience preferred.
• Demonstrate proficiency in documentation and clinical assessments.
• Maintain current BLS certification from AHA.
• Have a valid driver’s license and personal transportation for patient visits.
• Ability to communicate effectively and work independently in a home health environment.
