Job Summary:
We are seeking a licensed Occupational Therapist to provide home health services in Berkeley and Oakland. The role involves conducting personalized patient visits, managing documentation, and ensuring quality care delivery in a home environment.

Responsibilities:

• Assess patient needs and develop individualized treatment plans in accordance with best practices and regulations
• Perform OASIS assessments accurately and efficiently for reimbursement and quality reporting
• Document clinical notes and progress reports promptly and thoroughly
• Coordinate with care teams and schedule visits based on patient requirements and travel logistics
• Manage travel between patient locations, maintaining adherence to mileage and visit schedules
• Participate in on-call rotations and team meetings as required

Qualifications:

• Hold a valid California occupational therapy license or in the process of obtaining one
• Possess at least 1 year of home health experience, with home health experience preferred
• Demonstrate proficiency in OASIS documentation and home health protocols
• Maintain current BLS certification from the American Heart Association
• Have a valid driver’s license with reliable personal transportation during employment
• Exhibit strong communication skills and timely documentation
