Job Details

Job Summary:
Provide skilled occupational therapy services to patients in a home health setting, ensuring quality care and adherence to clinical standards. Coordinate with team members to support patient recovery and independence.

Responsibilities:

• Prioritize and schedule start of care visits to ensure timely patient service.
• Supervise and collaborate with Occupational Therapy Assistants to maintain treatment standards.
• Complete and submit clinical notes and documentation within 48 hours of each visit.
• Communicate promptly with supervisors, intake, and office staff regarding patient progress and scheduling needs.
• Maintain compliance with documentation and clinical protocols as required.
• Report and document patient assessments and treatment plans accurately.

Qualifications:

• Valid California Occupational Therapist License.
• Minimum of one year experience in Home Health therapy.
• Strong organizational skills and ability to prioritize multiple visits.
• Excellent communication and documentation proficiency.
• Ability to work independently within a team environment.
• Basic knowledge of Kinnser documentation system.
