Job Summary:
Seeking a dedicated Physical Therapist to provide skilled rehabilitation services to patients in a skilled nursing facility. The therapist will evaluate, develop, and implement treatment plans to improve patient outcomes and quality of life.

Responsibilities:

• Assess patient condition and develop individualized treatment plans.
• Administer therapeutic exercises and modalities to promote recovery.
• Document patient progress and adjust treatments accordingly.
• Collaborate with care teams to ensure coordinated patient care.
• Maintain compliance with licensing and facility standards.

Qualifications:

• Hold an active Arizona state license as a Physical Therapist.
• Proven experience working in a skilled nursing or similar setting.
• Demonstrate strong clinical assessment and treatment skills.
• Possess excellent communication and collaboration abilities.
• Ability to work five days per week, with a 32-hour guaranteed schedule.
