Job Summary:

Seeking a skilled Physical Therapy Assistant to provide home health therapy visits, supporting patients in their recovery while adhering to agency standards. The role involves traveling to patient homes, documenting care, and participating in periodic office meetings to ensure quality outcomes.

Responsibilities:

• Perform home health therapy visits for adult patients, including start-of-care and follow-up assessments.
• Document patient progress accurately within required timeframes and update EMR systems such as HCHB and PointCare.
• Travel up to 75 miles from the home clinic to patient locations, managing a caseload of 5-6 patients per day.
• Participate in periodic office meetings and collaborate with the healthcare team to deliver comprehensive patient care.
• Maintain current CPR and BLS certifications and ensure compliance with travel requirements and documentation standards.
• Adapt to different schedules, including standard five-day workweeks or Baylor shifts, based on assignment needs.

Qualifications:

• Possess a valid state license as a Physical Therapy Assistant with the ability to start immediately.
• Minimum of 1 year of recent adult home health or hospice experience.
• Demonstrate proficiency in OASIS, HomeCare HomeBase, or PointCare EMR systems.
• Have reliable transportation, a valid driver’s license, and current liability insurance.
• Ability to travel up to 75 miles and accept the responsibility of driving to multiple patient locations daily.
• Strong documentation skills and adherence to agency and healthcare standards.
