Job Summary:

Seeking an experienced LPN to provide compassionate care in a Discharge Lounge setting. This role involves managing patient discharge processes, charting, and supporting nursing staff to ensure smooth patient transitions.

Responsibilities:

• Administer oral, IM, SQ, NG/G-tube medications, and provide IV therapy duties utilizing pumps.
• Perform vital signs, pain assessments, intake/output monitoring, and blood glucose checks.
• Assist with wound care, Foley catheter insertion, specimen labeling, and oxygen administration.
• Communicate patient status changes accurately to RN/MD and support patient transfer procedures.
• Maintain thorough documentation in EPIC electronic health records.
• Float within skillset to cover medical patient units as needed.

Qualifications:

• Hold an active New York Single State LPN license.
• Demonstrate at least 1 year of recent acute care experience.
• Possess CPR/BLS certification.
• Be proficient in vital signs, medication administration, and basic wound care.
• Experience with telemetry monitoring, 12-lead EKG, and isolation precautions preferred.
• Ability to work 10-hour shifts and support every other holiday.
