Job Summary:
Seeking a qualified Occupational Therapist to provide patient care in a home health setting. The ideal candidate will have recent clinical experience and proficiency with specialized software tools.

Responsibilities:

• Administer comprehensive occupational therapy assessments and develop individualized treatment plans.
• Implement therapy interventions focused on enhancing patients’ daily living and functional abilities.
• Document clinical notes and progress reports accurately using HCHB software.
• Collaborate with healthcare team members to coordinate patient care.
• Maintain compliance with all healthcare regulations and safety standards.
• Manage caseload efficiently within scheduled hours, ensuring timely patient visits.

Qualifications:

• Hold a valid state licensure as an Occupational Therapist.
• Possess a minimum of 2 years clinical experience, including 3 months in the specialty.
• Have at least one year of home health experience as an OT.
• Be proficient with HCHB and other relevant documentation systems.
• Hold current AHA BLS certification.
• Be 21 years or older and demonstrate strong communication skills.
