Job Summary:

Provides comprehensive physical therapy services to evaluate, treat, and rehabilitate patients with physical functioning disorders, wounds, or other related conditions. Develops individualized care plans utilizing evidence-based practices to improve patient outcomes and promote recovery.

Responsibilities:

• Assess patients' physical conditions using standardized assessments and clinical judgment to determine appropriate treatment strategies.
• Implement therapeutic interventions such as manual techniques, ambulation, exercises, modalities, and wound care to facilitate recovery.
• Monitor and document patient progress, adjusting treatment plans based on clinical outcomes and regulatory guidelines.
• Communicate effectively with patients, families, caregivers, and healthcare team members regarding treatment goals and progress.
• Maintain accurate and complete documentation of all clinical activities, treatments, and discharge summaries in electronic health records.
• Supervise and support Physical Therapist Assistants, rehabilitation aides, and students to ensure quality care delivery.

Qualifications:

• Possess at least 1 year of physical therapy experience; new graduates with Level II experience are acceptable.
• Hold a valid Massachusetts State Physical Therapist license.
• Demonstrate proficiency in outpatient, LTC, or rehabilitation settings.
• Strong knowledge of evidence-based treatment techniques and regulatory compliance.
• Excellent communication and clinical documentation skills.
