Job Summary:

Provides skilled physical therapy services to patients in a home health setting, assessing their needs and developing individualized treatment plans to improve mobility and reduce pain.

Responsibilities:

• Evaluate patients' physical conditions and develop tailored treatment plans.
• Deliver therapy services during home visits, focusing on functional improvement.
• Document patient progress and treatment outcomes accurately within required timeframes.
• Participate in periodic office meetings to coordinate care and review caseload.
• Travel up to 75 miles to patient homes, ensuring timely and efficient visits.
• Maintain current licensure, CPR, and BLS certifications, and adhere to agency policies.

Qualifications:

• Hold a valid Physical Therapist license in the state of practice.
• Minimum of 1 year of recent adult home health therapy experience.
• Proficient in documentation systems such as HCHB and HomeCare HomeBase.
• Valid driver’s license, reliable transportation, and current insurance.
• Must be comfortable traveling up to 75 miles from the clinic.
• Ability to complete documentation within 24 hours of patient encounters.
