Job Summary:
Provide specialized physical therapy services to patients in a home health setting, promoting recovery and improving mobility. Ensure compliance with clinical standards and documentation requirements to deliver quality patient care.

Responsibilities:

• Evaluate patients' physical conditions to develop personalized treatment plans.
• Administer therapeutic interventions based on individual needs and goals.
• Document patient progress and maintain accurate medical records in compliance with industry standards.
• Collaborate with healthcare team members to coordinate care and optimize outcomes.
• Maintain current licensure and certifications, including BLS and OASIS expertise.
• Adhere to travel and scheduling policies, including on-call rotations and mileage tracking.

Qualifications:

• Hold a valid California Physical Therapist license upon submission.
• Possess at least 1 year of Home Health or outpatient therapy experience.
• Demonstrate proficiency with OASIS documentation.
• Have a current CPR certification from AHA.
• Maintain a valid driver’s license and personal transportation for travel between patient sites.
• Be able to work independently within established schedules, including day shifts and on-call duties.
