Job Summary:
Provide skilled occupational therapy services to patients in a skilled nursing facility, focusing on improving functional independence and quality of life. Collaborate with the interdisciplinary team to develop and implement personalized treatment plans.

Responsibilities:

• Evaluate patients' conditions and develop appropriate treatment protocols.
• Administer and monitor therapy sessions based on individual patient needs.
• Document patient progress and treatment outcomes in facility-approved systems.
• Collaborate effectively with team members to ensure holistic patient care.
• Maintain compliance with all licensing regulations and facility policies.
• Adapt to various clinical situations, including float coverage at sister facilities.

Qualifications:

• Hold an active California OT license or be in process under the 60-day rule.
• Possess experience in skilled nursing facilities or similar settings preferred.
• Demonstrate strong documentation and time-management skills.
• Exhibit a flexible, collaborative approach to patient care and team dynamics.
