Job Summary:

Provide comprehensive occupational therapy assistance to patients in a skilled nursing facility, supporting recovery and functional improvement. Collaborate with the therapy team to implement individualized treatment plans and ensure patient progress.

Responsibilities:

• Assist occupational therapists in conducting patient assessments and implementing therapy plans.
• Administer therapeutic activities and interventions under supervision, documenting patient responses.
• Maintain detailed records of patient progress using Rehab Optima (NetHealth) system.
• Adhere to all safety and clinical protocols to ensure patient and staff safety.
• Support patient engagement and motivation throughout treatment sessions.
• Keep accurate and timely documentation in compliance with regulatory standards.

Qualifications:

• Hold an active California license as an Occupational Therapy Assistant.
• Demonstrate SNF (Skilled Nursing Facility) experience with recent exposure to rehab therapy.
• Possess knowledge of Rehab Optima (NetHealth) therapy documentation system.
• Exhibit excellent communication and teamwork skills.
• Maintain current CPR and First Aid certification.
• Show commitment to patient care standards and confidentiality.
