Job Summary:
Seeking a dedicated Occupational Therapy Assistant (COTA) to provide patient-centered rehabilitative care in a skilled nursing facility. The COTA will support licensed Occupational Therapists in enhancing residents’ functional independence through personalized interventions.

Responsibilities:

• Implement occupational therapy treatment plans tailored to each resident’s needs.
• Communicate effectively with Occupational Therapists and interdisciplinary care teams.
• Maintain compliance with facility policies, Medicare guidelines, and regulatory standards.
• Complete comprehensive documentation in Net Health according to facility standards.
• Assist with patient assessments and progress tracking.
• Collaborate with team members to optimize patient outcomes.

Qualifications:

• Active OTA License required.
• Previous SNF experience preferred; mentorship available for those with 0-1 year experience.
• Demonstrate strong documentation and time-management skills.
• Possess flexible and collaborative work ethic.
• Ability to adapt to a dynamic healthcare environment.
