Job Description
Job Summary:
Provides specialized evaluation and rehabilitation services for patients with communication, cognitive, or swallowing disorders. Develops and implements treatment plans aligned with regulatory standards and evidence-based practices to improve patient outcomes.
Responsibilities:
- Conduct comprehensive assessments using standardized tools to identify patient needs.
- Develop and implement individualized treatment plans based on patient evaluations.
- Monitor patient progress and modify treatment approaches accordingly.
- Document all clinical activities, outcomes, and interventions in electronic health records.
- Educate and counsel patients, families, and caregivers on strategies to manage speech and swallowing disorders.
- Collaborate effectively with healthcare team members regarding patient status and care plans.
Qualifications:
- Hold current state licensure as a Speech-Language Pathologist.
- Possess a CCC-SLP certification from ASHA.
- Have at least 1 year of clinical experience as an SLP; new graduates with Level II experience accepted.
- Demonstrate knowledge of regulatory requirements and documentation standards.
- Experience in LTC or rehab settings preferred.
