Job Summary:
A Skilled Nursing Facility in Taos, NM is seeking a dedicated Physical Therapist to join their rehabilitation team. The role involves providing comprehensive patient evaluations, administering daily treatments, and managing discharges, ensuring optimal patient care and recovery.

Responsibilities:

• Conduct patient evaluations and develop personalized treatment plans.
• Administer daily therapies according to established protocols.
• Document patient progress and maintain accurate clinical notes.
• Follow facility policies and CRG P & P guidelines.
• Attend interdisciplinary team meetings to coordinate patient care.
• Adhere to productivity and attendance standards to ensure consistent coverage.

Qualifications:

• Hold a valid Physical Therapist license in New Mexico.
• Demonstrate prior experience in skilled nursing or rehab settings.
• Possess strong clinical assessment and treatment skills.
• Exhibit excellent communication and teamwork abilities.
• Be available to work in a facility setting with guaranteed hours.
• Comply with dress code of black scrubs and professional standards.
