Job Summary:
Provide skilled nursing care to patients in an outpatient rehab setting, ensuring treatment plans are effectively implemented. Collaborate with interdisciplinary teams to support patient recovery and health outcomes.

Responsibilities:

• Assess patient needs and develop individualized care plans.
• Administer therapies and treatments as prescribed by physicians.
• Monitor patient progress and adjust care plans accordingly.
• Document clinical assessments and interventions accurately.
• Educate patients and families on recovery processes.
• Maintain compliance with facility policies and safety standards.

Qualifications:

• Valid nursing license (RN or LPN) applicable to the state.
• Minimum of one year of relevant clinical experience preferred.
• Strong knowledge of rehabilitation therapies and protocols.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• CPR and BLS certification required.
