Job Summary:
Provide skilled transitional nursing and rehabilitation services within a 73-bed facility focused on rapid recovery and returning patients home. Deliver high-quality patient care in a supportive environment, emphasizing efficiency and rehabilitation success.

Responsibilities:

• Assess patients' needs and develop individualized care plans.
• Administer medications and treatments following physician orders.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Educate patients and families on post-discharge care and recovery.
• Document patient progress accurately in compliance with regulatory standards.
• Ensure safety protocols are maintained to promote a secure environment.

Qualifications:

• Hold a valid nursing license (RN preferred).
• Demonstrate experience in skilled nursing or rehabilitation settings.
• Possess strong patient assessment and documentation skills.
• Exhibit excellent communication and teamwork abilities.
• Be capable of managing a diverse caseload efficiently.
• Have knowledge of Medicare, Medicaid, and private insurance processes.
