Job Summary:
Seeking a qualified healthcare professional to provide comprehensive care at a CCRC facility featuring over 200 units, including Skilled Nursing Facility (SNF) and Assisted Living/Independent Living (AL/IL) services. The role offers a dynamic environment with diverse patient needs, focusing on quality resident outcomes.

Responsibilities:

• Deliver direct patient care in accordance with care plans and regulatory standards.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor patient conditions, noting changes and reporting to necessary team members.
• Collaborate with interdisciplinary team to develop and update individual care plans.
• Maintain accurate documentation of patient care and progress.
• Participate in weekend rotations and ensure continuous coverage during scheduled shifts.

Qualifications:

• Hold a current healthcare license relevant to the role.
• Demonstrate experience working in nursing, assisted living, or skilled nursing environments.
• Possess strong clinical skills with attention to detail.
• Exhibit excellent communication abilities and teamwork skills.
• Show flexibility to work Monday through Friday with weekend rotations as required.
• Be committed to providing compassionate, resident-focused care.
