Job Details

Job Summary:
Provide outpatient or inpatient physical therapy services to patients in a skilled nursing facility, assessing and developing treatment plans to improve mobility, strength, and overall functional ability.

Responsibilities:

• Evaluate patients’ physical conditions and establish individualized therapy goals.
• Develop and implement therapeutic treatment plans based on patient needs.
• Collaborate with interdisciplinary teams to coordinate patient care.
• Monitor and document patient progress and modify treatments as necessary.
• Educate patients and families on therapeutic exercises and injury prevention.
• Maintain accurate and timely clinical documentation in compliance with facility standards.

Qualifications:

• Hold a current state license or certification as a Physical Therapist.
• Possess solid knowledge of therapeutic techniques and modalities.
• Demonstrate excellent clinical assessment and problem-solving skills.
• Have previous experience working in a skilled nursing facility or similar environment.
• Ability to work effectively within a team and communicate clearly.
