Job Summary:
Provide occupational therapy services to patients in a skilled nursing facility, supporting their recovery and enhancing their functional independence. Collaborate with a multidisciplinary team to develop and implement personalized treatment plans.

Responsibilities:

• Assess patients’ clinical conditions to determine their treatment needs.
• Develop and execute individualized therapy plans aligned with patient goals.
• Document patient progress and adjust interventions accordingly.
• Educate patients and family members on therapeutic exercises and techniques.
• Collaborate with healthcare team members to coordinate care strategies.
• Comply with all regulatory and safety standards in therapy delivery.

Qualifications:

• Valid license or certification as an Occupational Therapist.
• Proven experience working in a skilled nursing or similar clinical setting.
• Strong assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively within a multidisciplinary team.
• Knowledge of relevant healthcare regulations and documentation standards.
