Job Summary:
Seeking a dedicated Occupational Therapist to join a skilled rehab team in a skilled nursing facility. The role involves conducting evaluations, delivering daily treatments, and managing discharges for a diverse patient population.

Responsibilities:

• Perform comprehensive patient evaluations to develop personalized treatment plans.
• Deliver daily occupational therapy treatments in accordance with established protocols.
• Monitor patient progress and adjust therapy plans as needed to optimize outcomes.
• Coordinate discharges by assessing patient progress and providing appropriate recommendations.
• Follow facility policies, including CRG P & P, and maintain a 90% productivity rate.
• Document all patient interactions accurately and promptly in compliance with regulatory standards.

Qualifications:

• Possess active Ohio Occupational Therapist license or equivalent.
• Prior experience in a skilled nursing facility setting strongly preferred.
• Demonstrate excellent clinical assessment and treatment planning skills.
• Maintain punctuality and adherence to dress code: black scrubs.
• Ability to work collaboratively within a multidisciplinary team.
• Present in the building during scheduled hours to ensure guaranteed hours.
