Job Details

Job Summary:
Provide outpatient physical therapy services to patients in a home health setting. Deliver skilled therapy interventions, evaluate patient progress, and collaborate with team members to optimize patient outcomes.

Responsibilities:

• Assess patients’ physical condition and develop individualized treatment plans.
• Administer therapeutic exercises, manual therapy, and modalities as prescribed.
• Document patient progress and treatment activities accurately within industry-standard EMR systems.
• Educate patients and caregivers on therapeutic routines and injury prevention techniques.
• Coordinate with team members to ensure seamless delivery of care and transition planning.
• Comply with all state and federal regulations, including on-call rotations and documentation standards.

Qualifications:

• Hold a valid, current state license to practice as a Physical Therapist.
• Have at least 1 year of recent adult home health experience.
• Possess CPR/BLS certification.
• Be proficient in EMR systems such as HomeCare HomeBase, Oasis, and Outlook.
• Have reliable transportation, a valid driver’s license, and agency-required liability insurance.
• Willing to travel up to 75 miles from the primary clinic location.
