Job Summary:
Seeking a licensed Physical Therapist to provide specialized rehabilitative services in a skilled nursing facility. The role involves evaluating patients, developing treatment plans, and collaborating with a multidisciplinary team to optimize patient outcomes.

Responsibilities:

• Assess patients' physical conditions and develop individualized treatment plans.
• Implement therapeutic interventions to improve mobility, strength, and functional abilities.
• Document patient progress and adjust treatments accordingly.
• Educate patients and family members on post-treatment care and exercises.
• Collaborate with nursing staff and physicians to coordinate care plans.
• Maintain compliance with all regulatory and facility standards.

Qualifications:

• Hold an active state license as a Physical Therapist.
• Possess a degree from an accredited physical therapy program.
• Have prior experience working in a skilled nursing or long-term care setting.
• Demonstrate excellent communication and teamwork skills.
• Be able to evaluate and adapt treatment plans based on patient needs.
