Job Summary:
The Nurse Care Coordinator provides comprehensive nursing care with a focus on breast and cervical cancer prevention, screening, and diagnostic services. This role involves patient assessment, care planning, and collaboration with interdisciplinary teams to promote optimal health outcomes and patient-centered care.

Responsibilities:

• Perform thorough patient assessments and develop individualized care plans.
• Administer medications and treatments according to provider orders and organizational policies.
• Educate patients and families on care procedures, medications, and discharge instructions.
• Recognize and respond to changes in patient condition, escalating care as needed.
• Collaborate with healthcare providers and team members to coordinate follow-up and continuity of care.
• Document assessments, interventions, and outcomes accurately in the electronic health record.

Qualifications:

• Hold an Associate’s degree in Nursing from an accredited program.
• Possess an active, unrestricted Registered Nurse license in Alaska.
• Maintain Basic Life Support (BLS) certification.
• Complete case management certification within 2 years of hire.
• Demonstrate clinical competency within 3 months and every 3 years.
• Obtain required specialty certifications (e.g., PALS, NRP, TNCC) within six months of hire if applicable.
