Job Summary:
The Nurse Care Coordinator provides comprehensive, patient-centered nursing care in outpatient settings, supporting individuals and families across the continuum of care. This role involves clinical assessments, medication administration, patient education, and care coordination to promote optimal health outcomes.

Responsibilities:

• Perform thorough patient assessments and develop individualized care plans.
• Administer medications and treatments accurately according to provider orders and policies.
• Recognize changes in patient condition and initiate appropriate interventions or escalate care as needed.
• Educate patients and families on care plans, medications, and discharge instructions.
• Collaborate with providers and interdisciplinary teams to coordinate care, referrals, and follow-up services.
• Document assessments, interventions, and outcomes precisely in electronic health records and ensure regulatory compliance.

Qualifications:

• Hold an associate’s degree in Nursing from an accredited program.
• Possess a current, unrestricted Registered Nurse license in Alaska.
• Maintain Basic Life Support (BLS) certification.
• Demonstrate clinical competency within three months and ongoing every three years.
• Obtain all certification requirements relevant to practice area within six months of hire, including specialty-specific credentials if applicable.
• Exhibit knowledge of evidence-based nursing principles, medication safety, and infection prevention.
