Job Summary:
Provides comprehensive nursing care to patients in a skilled nursing or long-term care setting, ensuring adherence to established standards and protocols. Supervises and collaborates with nursing staff and interdisciplinary teams to deliver safe, quality patient care.

Responsibilities:

• Assess patients' health conditions and document relevant data.
• Develop individualized care plans based on patient assessments.
• Implement nursing interventions while coordinating care with healthcare team members.
• Educate patients and families regarding health management, treatments, and discharge planning.
• Supervise and delegate duties to nursing staff, ensuring proper care delivery.
• Evaluate patient outcomes and adjust care plans accordingly, maintaining safety and quality standards.

Qualifications:

• Current Registered Nurse (RN) license in the state of practice.
• Minimum of two years recent experience in LTC/SNF or acute care.
• Valid Basic Life Support (BLS) certification from an approved vendor.
• Knowledge of long-term care regulations and best practices.
• Strong clinical assessment and critical thinking skills.
