• Document patient progress accurately using electronic health records.
• Collaborate with a small, experienced rehab team to ensure optimal patient outcomes.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• Proven experience in assisted living or memory care environments preferred.
• Demonstrate strong assessment and treatment planning skills.
• Proficient in electronic documentation systems such as Net Health.
