Job Summary:
Provides skilled physical therapy services to patients within a hospital's Skilled Nursing Unit and potential acute care settings. Responsible for facilitating patient recovery and promoting functional independence through individualized treatment plans.

Responsibilities:

• Evaluate patients’ physical conditions and develop comprehensive treatment plans.
• Implement therapy interventions tailored to each patient's needs and recovery goals.
• Collaborate with multidisciplinary teams to coordinate patient care.
• Document patient progress and adjust treatment strategies accordingly.
• Supervise and guide rehab aides and PTA staff in therapy delivery.
• Maintain compliance with all certification and licensing requirements.

Qualifications:

• Hold a valid Physical Therapist license or a Temporary License through the West Virginia Board of Physical Therapists or a Physical Therapist Compact License.
• Possess BLS certification and familiarity with general certification standards.
• Demonstrate experience in outpatient, inpatient, or rehab therapy within acute or skilled nursing settings.
• Exhibit strong clinical assessment and treatment planning skills.
• Capable of functioning effectively across all hospital floors and units.
• Show excellent teamwork and communication abilities.
