Job Summary:
Provide skilled physical therapy services to patients in their homes, focusing on recovery and functional improvement. Ensure appropriate treatment plans are implemented and documented according to standards.

Responsibilities:

• Evaluate patient conditions and develop individualized treatment plans.
• Administer physical therapy interventions to promote mobility and independence.
• Document patient progress and treatment outcomes in EPIC and OASIS systems.
• Complete start of care assessments and perform revisits as needed.
• Collaborate with healthcare teams to coordinate patient care and updates.
• Maintain compliance with regulatory and organizational policies.

Qualifications:

• Recent and relevant home health experience required.
• Valid PT license in the state of New Jersey.
• Proficiency in EPIC electronic health record system.
• Experience with OASIS assessments essential.
• Strong communication and documentation skills.
• Ability to work independently in a home health setting.
