Job Summary:
Seeking a licensed Occupational Therapist to provide comprehensive therapy services in a Skilled Nursing Facility. The therapist will evaluate, treat, and discharge patients while adhering to facility policies and quality standards.

Responsibilities:

• Conduct initial assessments and develop individualized treatment plans for patients.
• Administer daily treatments and monitor patient progress.
• Document clinical notes and maintain accurate records in accordance with policies.
• Collaborate with the rehab team and other healthcare professionals to optimize care.
• Meet productivity goals and follow facility guidelines for treatment delivery.
• Ensure timely discharges and proper documentation for each patient.

Qualifications:

• Hold a valid Occupational Therapist license in the state of New Mexico.
• Possess prior SNF or skilled rehab experience preferred.
• Demonstrate strong clinical assessment and treatment skills.
• Maintain excellent communication and documentation skills.
• Be able to follow facility policies, including dress code and productivity standards.
• Be available to start within 2–3 weeks.
