Job Summary:
Provides comprehensive nursing care to residents in a Long Term Care facility, delivering assessments, treatments, and patient support with a focus on maintaining health and safety. Ensures communication with healthcare providers and records pertinent clinical information accurately.

Responsibilities:

• Perform nursing assessments, treatments, and medication administration according to care plans.
• Notify MD/NP of relevant patient updates and changes in condition.
• Assist with activities of daily living (ADLs) and provide specialized wound and tube care.
• Administer respiratory treatments such as nebulizers and CPAP, while managing urinary catheters and wound vacs.
• Document all patient care activities accurately and adhere to facility protocols.
• Support patient discharge and admission procedures, ensuring proper documentation and care continuity.

Qualifications:

• Hold an active Hawaii Registered Nurse license by the start of the assignment.
• Must be current or willing to apply for Hawaii RN licensure if not already licensed.
• Demonstrate clinical proficiency in long-term care practices and patient assessments.
• Possess excellent communication skills for liaising with medical professionals and patients.
• Be adaptable to varied shift schedules including days, evenings, nights, weekends, and holidays.
• Ability to perform multiple clinical duties with attention to detail and precision.
