Job Summary:

Seeking a dedicated Licensed Practical Nurse (LPN) to provide compassionate patient care in a long-term care or skilled nursing facility setting. The ideal candidate will ensure patient comfort and safety while supporting overall care plans.

Responsibilities:

• Administer medications and treatments accurately according to established protocols.
• Monitor patient vital signs and assess health status regularly.
• Document patient care activities and any changes in condition thoroughly.
• Collaborate with the healthcare team to develop and implement care plans.
• Educate patients and their families on medication and health management.
• Maintain compliance with facility policies and legal requirements, including ID and badge protocols.

Qualifications:

• Valid LPN license with current State licensure.
• Proven experience in LTC/SNF environments preferred.
• Ability to adhere to infection control and safety procedures, including COVID-19 vaccination requirements.
• Strong communication and interpersonal skills.
• Excellent observational skills and attention to detail.
• Capable of wearing a photo ID badge and complying with facility identification policies.
