Job Summary:
Our healthcare facility is seeking a dedicated Physical Therapist with home health experience to provide outpatient rehabilitation services. The ideal candidate will deliver patient-centered care, helping individuals regain mobility and improve their quality of life in a home setting.

Responsibilities:

• Evaluate patients' physical health and develop personalized treatment plans.
• Implement therapeutic interventions to improve mobility, strength, and function.
• Document patient progress and adjust treatments accordingly.
• Collaborate with healthcare team members to coordinate patient care.
• Adhere to facility policies, including attendance and documentation standards.
• Maintain up-to-date knowledge of clinical practices and compliance requirements.

Qualifications:

• Hold a valid state license with the ability to obtain or transfer it prior to starting.
• Possess at least 2 years of PT home health experience; 1-year experience may be considered.
• Demonstrate proficiency with EPIC documentation (preferred but not required).
• Successful completion of Cardiac Dysrhythmia and Moderate Sedation tests at orientation.
• Ability to float within scope to meet facility needs.
• Be able to work day shifts, Monday through Friday, with flexibility for occasional travel.
