Job Summary:
Provide skilled physical therapy services in an LTAC/Acute Care setting, focusing on patient recovery and functional improvement. Collaborate with the healthcare team to deliver high-quality, personalized care to diverse patient populations.

Responsibilities:

• Evaluate patients’ physical conditions and develop tailored treatment plans to promote mobility and recovery.
• Implement therapeutic interventions, including therapeutic exercises and manual techniques.
• Monitor patient progress and adjust treatment strategies accordingly.
• Document patient care and progress in compliance with facility and regulatory standards.
• Communicate effectively with patients, families, and interdisciplinary team members to ensure optimal care delivery.
• Adhere to all facility policies, including attendance and compliance requirements.

Qualifications:

• Active state license or pending license accepted prior to start date.
• Minimum of 2 years of relevant clinical experience; 1-year consideration on a case-by-case basis.
• Recent experience with EPIC EMR system preferred.
• Ability to pass required Cardiac Dysrhythmia and Moderate Sedation tests on first attempt.
• Strong knowledge of acute care and LTAC patient requirements.
• Flexibility to float within scope as needed; mileage paid.
