Job Summary:
A skilled Physical Therapist is sought to provide comprehensive rehabilitative care in a skilled nursing facility. The role involves evaluating patients, developing individualized treatment plans, and implementing therapeutic interventions to promote recovery and functional independence.

Responsibilities:

• Assess patients’ physical abilities and develop personalized treatment plans.
• Implement therapeutic exercises and modalities to improve mobility, strength, and coordination.
• Document patient progress and modify treatment strategies as needed.
• Collaborate with healthcare team to optimize patient outcomes.
• Educate patients and their families on outpatient care and injury prevention.

Qualifications:

• Current valid state licensure as a Physical Therapist.
• Minimum of at least 1 year of experience in a skilled nursing or rehabilitation setting.
• Strong knowledge of physical therapy techniques and modalities.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
