Job Summary:

We are seeking an experienced Physical Therapist to provide home health services in Montgomery and upper PG County. The role involves delivering patient-centered care across adult populations, with an emphasis on documentation accuracy and scope of practice adherence.

Responsibilities:

• Evaluate patients' physical conditions and develop individualized treatment plans.
• Administer therapeutic interventions to improve patient mobility and function.
• Document patient progress and treatment outcomes accurately.
• Collaborate with the healthcare team to coordinate care effectively.
• Maintain compliance with all licensing, certification, and facility policies.
• Adhere to schedule and visit expectations, ensuring six visits per day.

Qualifications:

• Hold an active Maryland or Compact State license as a Physical Therapist.
• Possess a minimum of 1 year of clinical experience in home health.
• Demonstrate home health experience with HCHB preferred.
• Be certified in Basic Life Support (BLS).
• Have availability for M-F daytime shifts and a flexible schedule.
• Able to complete required orientation modules via WorkDay.
