Job Summary:
Provide comprehensive physical therapy services within the legal scope of practice, ensuring patient-centered care and adherence to regulatory standards. Deliver evaluations, develop treatment plans, and administer skilled interventions to promote patient recovery and functional independence.

Responsibilities:

• Conduct initial assessments, progress notes, and periodic re-evaluations in compliance with Medicare, state, and agency standards.
• Develop, implement, and modify individualized care plans aligned with physician instructions and patient needs.
• Deliver targeted therapy interventions, including maintenance and mobility programs, within the scope of practice.
• Supervise Physical Therapist Assistants (PTAs) and Home Health Aides in accordance with state practice acts.
• Collaborate with physicians and interdisciplinary team members to optimize treatment plans and patient outcomes.
• Educate patients and caregivers on home exercise programs, mobility techniques, and safety strategies to enhance independence.

Qualifications:

• Graduate from an accredited physical therapist program approved by the Commission on Accreditation in Physical Therapy Education or equivalent.
• Valid license to practice as a Physical Therapist in the state of practice.
• Current CPR certification.
• Ability to exercise sound clinical judgment and work independently across varied settings.
• Strong communication skills and sensitivity to diverse patient backgrounds.
• Basic computer skills; willingness to learn EMR and OASIS documentation.
