Job Details

Job Summary:
Provide individualized physical therapy services to patients in a home health setting, focusing on patient mobility, rehabilitation, and functional independence. Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.

Responsibilities:

• Assess patient needs and develop tailored treatment plans to improve mobility and function.
• Administer therapeutic modalities, including splints, orthoses, prostheses, wheelchairs, and ADL equipment.
• Document patient progress and modify treatment plans based on clinical evaluations.
• Educate patients and caregivers on therapeutic exercises and equipment usage.
• Ensure compliance with all state and federal licensure and certification requirements.
• Maintain accurate clinical documentation and complete OASIS assessments as required.

Qualifications:

• Current Massachusetts Physical Therapist license required.
• Minimum of 2 years’ experience in acute hospital care and home health settings mandatory.
• Proven proficiency with OASIS documentation and home health protocols.
• Valid Driver’s License and own vehicle required for travel.
• Ability to work independently with minimal supervision.
• Home Health Certified preferred.
