Job Summary:

Seeking a Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) for a contract assignment in a long-term care and skilled nursing facility. This role involves providing high-quality nursing care to residents in a fast-paced environment.

Responsibilities:

• Administer prescribed medications and treatments accurately and safely.
• Monitor residents' health status and document changes meticulously.
• Collaborate with interdisciplinary teams to develop and update care plans.
• Ensure compliance with facility policies and regulatory standards.
• Maintain a clean, safe, and therapeutic environment for residents.
• Communicate effectively with residents, families, and staff to facilitate resident well-being.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Minimum of 1 year of experience in LTC or SNF settings.
• Proven ability to handle multiple residents and prioritize tasks.
• Demonstrate strong communication and teamwork skills.
• Ability to adhere to infection control protocols and safety policies.
• Must have completed the original or updated COVID-19 vaccination or present a valid exemption.
