Job Summary:

Seeking an experienced Physical Therapist to provide home health therapy services, including patient assessments, treatment planning, and therapy execution. Candidate will work primarily through home visits, supporting adult patients in their recovery and maintaining functional mobility.

Responsibilities:

• Evaluate patient conditions and develop individualized treatment plans to improve mobility and function.
• Administer therapy interventions including exercises, manual therapy, and modalities.
• Document patient progress and update care plans in the electronic medical record system.
• Coordinate with care teams to ensure seamless delivery of home health services.
• Travel to patients’ homes within designated territory, covering up to 75 miles from the clinic.
• Participate in periodic office meetings and mandatory documentation completion within 24 hours.

Qualifications:

• Hold a current and active state licensure as a Physical Therapist.
• Possess at least 1 year of recent adult home health therapy experience.
• Be proficient in home health EMR systems, particularly HomeCare HomeBase.
• Demonstrate ability to travel up to 75 miles and manage a patient load of 5-6 visits per day.
• Maintain current driver’s license, auto insurance, and reliable transportation.
• Have valid CPR/BLS certification.
