Job Summary:
Provide comprehensive physical therapy services to patients in a home health setting, supporting recovery and mobility through personalized treatment plans. Ensure adherence to clinical protocols while managing patient visits and documentation efficiently.

Responsibilities:

• Assess patients' physical conditions and develop individualized treatment plans.
• Deliver therapeutic interventions during home visits to promote recovery and function.
• Document patient progress accurately within 24 hours of each session.
• Coordinate with team members and attend periodic office meetings.
• Travel up to 75 miles between patient homes and clinic locations as needed.
• Maintain current licenses, certifications, and liability insurance as required.

Qualifications:

• Hold a valid state license as a Physical Therapist.
• Have at least one full year of recent adult home health therapy experience.
• Proficient in documentation and using EMR systems such as HomeCare HomeBase (HCHB).
• Possess reliable transportation, a valid driver’s license, and auto insurance.
• Be willing to travel up to 75 miles from the primary care location.
• Available for day shifts, with flexible weekend or on-call rotations as required.
