Job Summary:
Provides specialized physical therapy services to improve patients' physical, social, and mental functioning through personalized treatment plans. Collaborates with interdisciplinary teams to ensure optimal patient outcomes and quality care.

Responsibilities:

• Conduct comprehensive evaluations to assess patient needs and develop individualized treatment plans.
• Utilize evidence-based clinical techniques and tools to deliver effective therapy interventions.
• Establish and communicate care and discharge plans aligned with patient goals and recovery process.
• Complete accurate and timely clinical documentation to ensure compliance and continuity of care.
• Supervise and mentor assigned clinical assistants to maintain high standards of practice.
• Perform additional duties related to patient care and departmental objectives as assigned.

Qualifications:

• Hold a Doctor of Physical Therapy (DPT) degree; licensed in all applicable states of practice.
• Maintain valid Physical Therapy licensure and CPR/BLS certification from approved vendors.
• Possess a valid Driver’s License and be at least 21 years of age.
• Demonstrate strong clinical assessment and treatment skills with a patient-centered approach.
• Exhibit excellent communication and documentation abilities.
