Job Summary:
Provide outpatient physical therapy services to patients in a skilled nursing facility, supporting recovery and mobility improvements through evidence-based interventions.

Responsibilities:

• Assess patients' physical conditions and develop personalized treatment plans.
• Implement therapy techniques to improve strength, endurance, and functional mobility.
• Document patient progress accurately and maintain compliance with facility policies.
• Collaborate with the therapy team and nursing staff to coordinate care.
• Follow facility protocols, including dress code and documentation standards.
• Ensure productivity standards are met consistently.

Qualifications:

• Valid State Physical Therapy Assistant license or credential.
• Previous experience in a skilled nursing or rehabilitation setting preferred.
• Ability to communicate effectively in English and Spanish.
• Demonstrate strong clinical skills and adherence to treatment plans.
• Maintain professional appearance with required dress code of black scrubs.
• Ability to work collaboratively within a team environment.
