Job Summary:

Assist patients in improving their mobility and functional independence through tailored therapeutic interventions. Collaborate with multidisciplinary teams to develop and implement effective treatment plans in a skilled nursing facility setting.

Responsibilities:

• Evaluate patients' physical conditions and develop individualized therapy plans.
• Administer therapeutic exercises and modalities to enhance mobility and strength.
• Monitor and document patient progress, adjusting treatments as needed.
• Educate patients and family members on exercises and strategies for ongoing improvement.
• Maintain accurate and timely clinical documentation in compliance with regulatory standards.
• Adhere to safety protocols and maintain a clean, safe therapy environment.

Qualifications:

• Hold a valid state license or certification as a Physical Therapist.
• Demonstrate strong clinical assessment and therapeutic skills.
• Possess excellent communication and interpersonal abilities.
• Maintain current CPR and BLS certifications.
• Prior experience in a skilled nursing or long-term care setting preferred.
• Ability to work collaboratively within a multidisciplinary team.
