Job Summary:
Assist licensed physical therapists in providing rehabilitative therapy services to patients in a skilled nursing facility setting. Ensure effective treatment delivery and patient progress monitoring under supervision.

Responsibilities:

• Collaborate with physical therapists to develop and implement treatment plans.
• Administer therapeutic exercises and modalities to patients.
• Monitor patient responses and document progress accurately.
• Maintain updated knowledge of SNF protocols and rehabilitation procedures.
• Ensure compliance with all regulatory and safety standards, including licensing requirements.
• Support the care team in delivering quality patient care.

Qualifications:

• Hold a current and active CA license as a Physical Therapy Assistant.
• Preferably possess SNF experience and familiarity with NetHealth (Rehab Optima).
• Demonstrate strong clinical skills in rehabilitative therapies.
• Exhibit excellent communication and interpersonal skills.
• Be detail-oriented with the ability to follow treatment plans precisely.
• Hold relevant certifications as required by state regulations.
